This trial evaluates a grocery program to help adults with diabetes or hypertension manage their health. Participants select foods to be delivered, and complete surveys.

Who is the study for?
This trial is for adults with Type 2 Diabetes or Hypertension who are on Medicaid, can get texts, speak English, control their diet, and won't move for a year. They must have an A1c of 7-12% or diagnosed hypertension and be facing food insecurity.Check my eligibility
What is being tested?
The study tests if getting weekly home deliveries of healthy foods through Instacart Fresh Funds helps manage Type 2 Diabetes or Hypertension. Participants will also complete surveys before and after the 12-week program.See study design
What are the potential side effects?
There may not be direct side effects from participating in this program; however, changes in diet could potentially cause digestive adjustments or allergic reactions to new foods introduced.
